Social Worker, Bereavement and Spiritual Support
Our hospice social workers provide emotional and practical support to patients and families, helping them navigate the challenges of end-of-life care. They assist with decision-making, financial concerns, and connect families to community resources.
In addition, our bereavement and spiritual care services offer comfort and guidance, addressing emotional and spiritual needs. Whether through counseling, support groups, or spiritual guidance, we help families find peace and healing during this difficult journey.
Pain and Symptoms Management

Medications, Medical Equipment and Supplies
Related to hospice diagnosis
We provide all necessary medications, medical equipment, and supplies related to the hospice diagnosis to ensure comfort and care. This includes pain management medications, mobility aids, wound care supplies, and other essential equipment tailored to the patient’s needs.

Physical, Speech and Occupational Therapies

Levels of Hospice Care
Routine Home Care
The most common level, providing ongoing care in patient’s hime. Includes regular visits from the hospice team for symptom management, comfort and support for both the patient and family.
Continuous Care
Also called crisis care, this level provides extended nursing support at home during times of severe symptoms or when death is imminent. Hospice staff may remain for hours or overnight to manage symptoms and offer comfort, allowing families to focus on being present with their loved one.
Respite Care
Short-term inpatient care offered to give family caregivers a time-off while the patient continues to receive hospice care and support. It’s available in a skilled nursing facility, hospice inpatient unit, or hospital for up to 5 days during each certification period.
General Inpatient Care
For short-term management of severe pain or symptoms that require intensive management and cannot be controlled at home. Delivered in an inpatient setting at a medical facility like a hospital or skilled nursing facility.

Medical Director
Oversees care plans, ensuring quality and adherence to best practices and collaborates with hospice care team and patient’s primary physician.
Attending Physician
The attending physician oversees the patient’s medical care, focusing on comfort and quality of life. They collaborate with the hospice team to manage symptoms, adjust treatments, and guide families in making informed decisions.
Registered Nurse
The registered nurse in hospice care provides expert medical support, managing symptoms, administering medications, and ensuring comfort. They work closely with patients and families, offering guidance, education, and emotional support throughout the hospice journey.
Licensed Vocational Nurses
Licensed Vocational Nurses (LVNs) provide essential care under the supervision of registered nurses, assisting with symptom management, medication administration, and daily patient needs. They offer compassionate support, ensuring comfort and dignity for patients and families during this critical time.
Hospice Aides
Hospice aides provide personal, hands-on care, assisting with daily activities such as bathing, grooming, and mobility. They help ensure patient comfort and dignity while offering families much-needed support and respite during the caregiving process.
Social Workers
Hospice social workers offer emotional support and practical assistance to patients and families, helping them navigate end-of-life decisions, cope with grief, and access community resources. They provide guidance on legal, financial, and emotional concerns, ensuring families have the support they need during this challenging time.
Chaplains
Bereavement Coordinators
Bereavement coordinators offer support to families coping with the loss of a loved one. They provide grief counseling, resources, and emotional guidance, helping families navigate the mourning process and find healing after the passing of a patient.
